If the cap blew off it would probably jam the action. Spent caps will sometimes come off & lock up the cylinder. Hence the point-it-up-and-shake between rounds.

That flash hole is pretty small, path of least resistance would be out the big end. I'm thinking the pressure spike is over before much gas has a chance to vent thru the tiny flash hole.

What does my experience with recoil and a PCC have to do with your SA revolver? I wrote it in 2 different paragraphs because it's 2 different ideas. I also indicated that when I read the subject I thought, "BS" and not when I read the explanation. To that I replied "Bizarre." I'll need to be more obvious next time.

BP is a lot weaker than modern. That is probably part of it.
The whole thing sounds ultra rare. Just the right combination of malfunctions kind of rare.
Before I modified it my makarov would very rarely go full auto for 2-3 shots if you pulled and held the trigger down with extreme force. Its just a machine, sometimes stuff does not catch and lock when it should.
